Carolina Well is partnering with communities across the state to better coordinate services
in the communities in which cancer survivors live and work. Current community partners
include Greensboro AHEC Area Health Education Center, CommWell Health Center in
Sampson County, New Hanover Health Network in Wilmington and East Carolina University in
Greenville.
These partnerships bring more cancer survivorship services to communities by serving as a
local resource for cancer survivorship needs, helping you find services, connecting you
with people who have been down the same path, coordinating survivorship programs, offering
local educational and training programs for cancer survivors and caregivers including Peer
Connect, as well as offering continuing education for health professionals. There is no
extra cost for these services. These services are available to you and your loved ones at
any time during the cancer journey.
CWH strives to provide care to the local community in a culturally appropriate,
affordable and dignified manner. CWH provides care for community members and farm workers
in Sampson, Harnett, Johnston, Bladen and Pender Counties.
Services available at CWH include pre-natal care, WIC services, physical examinations,
minor surgical procedures, immunizations, X-ray services, lab services, nutrition
counseling, HIV testing and counseling, cancer survivorship information and much more. A
full service dental clinic, a pharmacy, a migrant head-start program and a residential
substance abuse treatment center are also on site.
Patients can be seen by appointment or on a walk-in basis and charges are on a sliding
scale. CWH accepts most private insurances as well as Medicaid and Medicare. All center
services are provided in both English and Spanish.

Greensboro AHEC, is an affiliate of the North Carolina Area Health Education Centers Program
of The UNC School of Medicine, and a department of Moses Cone Health Systems. AHEC's mission
is to meet the state's health and health workforce needs by providing educational programs in
partnership with academic institutions, health care agencies, and other organizations
committed to improving the health of the people of North Carolina. The Greensboro AHEC serves
an eight county area that includes Alamance, Caswell, Chatham, Guilford, Montgomery, Orange,
Randolph and Rockingham counties. As a partner of Carolina Well, Greensboro AHEC is working
with other community organizations to provide survivorship services that meet the needs of
local cancer survivors in the area.

The Zimmer Cancer Center, located in Wilmington, NC, is dedicated to the diagnosis,
treatment and support of people with cancer. The Cancer Center was designed by a team of
cancer patients, doctors, nurses and administrators, and is supported through donations
from the entire community. Zimmer Cancer Center not only offers outpatient services,
treatment, education and support areas, it is designed to help lift the spirits of patients
and their families in the surrounding area.
The center offers:
- cancer treatments — including chemotherapy, radiation
- complementary therapies — including therapeutic touch, massage, dance/movement, yoga, expressive arts and musical therapy
- mobile mammography services for southeastern North Carolina

The Leo W. Jenkins Cancer Center is part of East Carolina University located in Greenville,
NC. The Center offers a multidisciplinary approach to cancer care featuring
state-of-the-art therapies, compassionate care, expert second opinions, support services,
nutrition counseling, holistic care, and clinical trials. Multidisciplinary conferences
such as the breast tumor board, thoracic oncology clinic, pediatric tumor board, head and
neck tumor board, gamma knife conference, and melanoma tumor board allow physicians from
different specialties to reach a consensus as to the optimal plan of action for each
patient.
Leo W. Jenkins Cancer Center is Carolina Well's newest community partner. We are in the
